This form serves several important purposes:

It helps us confirm your attendance so we can dedicate our time and resources to you.
It ensures that all items you may try on during your visit are sanitized and ready for use, prioritizing your safety and well-being.
Here’s how it works:
You’ll receive your Fit Form automatically 48 hours before your scheduled appointment via text or email, whichever you prefer.
Please return the completed form no later than 12 hours before your appointment to confirm your spot.
If we don’t receive your Fit Form by the deadline, you’ll get two reminders. Unfortunately, if we still don’t hear from you, your appointment will be automatically canceled.
