To help us prepare for your appointment and provide you with the best possible service, we ask all clients to complete a Fit Form before their visit.

This form serves several important purposes:

• It allows us to prepare for your specific needs, ensuring we have everything ready for your appointment.
• It helps us confirm your attendance so we can dedicate our time and resources to you.
• It ensures that all items you may try on during your visit are sanitized and ready for use, prioritizing your safety and well-being.

Here’s how it works:

• You’ll receive your Fit Form automatically 48 hours before your scheduled appointment via text or email, whichever you prefer.
• Please return the completed form no later than 12 hours before your appointment to confirm your spot.
• If we don’t receive your Fit Form by the deadline, you’ll get two reminders. Unfortunately, if we still don’t hear from you, your appointment will be automatically canceled.
